Chase Albert Simmons
Chase Albert Simmons, age 45, cherished husband, father, son, brother, uncle, and friend, passed away unexpectedly, on Wednesday, 11/12/2025, surrounded by his loved ones.
Chase is survived by his family, comprised of the people he treasured most in this world. His wife of 10 years, Deeanne, an RN; sons, Payton, a Data Analyst, Cooper, studying plumbing, and Teague, a junior at Onalaska High School and a nationally ranked Powerlifter; daughter, Naveyah, a senior at UW Green Bay, majoring in Marketing; their mother, Heather (Onalaska); mother, Janet Hall (Elroy); father, Albert Simmons (Ventura, CA); sister, Cory Smith (Robert, Morgan and Parker); faithful companion, Maya; and countless other relatives, co-workers, and friends.
Chase graduated from Winona State University in 2004, with a degree in Criminal Justice, and his career path centered on corrections, security, and his true calling – mentorship. For the last year and a half of his life, he worked as a Psychiatric Care Technician-Advanced (PCT-A,) at Sand Ridge Secure Treatment Center (SRSTC,) where, in a short period of time, he made a lasting impact on those with whom he worked closely. Chase was a valued SRSTC team member, who consistently exhibited professionalism, initiative, respect, and compassionate care, and he will be greatly missed by his work family.
To know Chase, was to know he loved watching, researching, playing, and discussing sports, of any kind really, participating in fitness challenges/competitions with his wife, and attending every event in which his children participated, as their strongest and loudest super-fan.
But, if you really knew Chase, it comes as no surprise what he prioritized and enjoyed above all, was time spent with his family, whatever that looked like. It offers some comfort, Chase can rest peacefully knowing the people he valued most, knew how much they were loved. When his children were asked what they will remember most about their dad, they said, “Making sure we were safe wherever we went. He made it a point to tell us, ‘Have a good day,’ every day. And that he loves us, every day. Cheering us on for test day or a new job – he was always excited/happy/proud for us.”
Chase’s family was able to return this infinite love, staying by his side, in the ICU, for his last two weeks of life. He had the privilege of leaving this world, listening to his family share their favorite memories, photos, and stories, while ensuring he knew how deeply he is loved and how desperately he would be missed. Like the fierce protector he was, Chase went first, to prepare a place for his loved ones. And while they will miss him, endlessly, they look forward to the day they will see him again, get a big bear hug, turn on some of his favorite music, and catch him up, with everything he missed. (And because Chase was a lifelong Toronto Blue Jays fan, they will also wait until they’re reunited, to tell him the outcome of the 2025 World Series, Game 7.)
Earlier this year, when Chase was diagnosed with cancer, he told only his closest family members, as he did not want “pity” or “a lot of attention.” Ironically, and unbeknownst to him, Chase’s humor, humility, warmth, and approachability, courted attention wherever he went, leaving a positive and enduring impression, and all who knew him will miss him, forever.
Chase’s family would like to recognize the Gundersen Lutheran Medical Center Critical Care Unit (CCU) staff, for providing both he and his family with the most exhaustive and extraordinary care and compassion. They will remember your patience, grace, and support, always.
Chase’s family will hold a celebration of his life on Friday, November 28th, from 11:00 A.M. – 1:00 P.M., at The Gathering Place, 133 Mason Street, Onalaska, WI, 54650.

My beloved son,
There are no words big enough to describe the love I have for you or the pain of losing you. You were the greatest blessing of my life a son who brought pride, joy, and meaning into every day. I will carry your memory with me for the rest of my life, and I will love you for as long as I breathe.
Thank you for the laughter, the moments, the lessons, and the love. I am so grateful I got to be your father. My heart is broken, but it is full of you.

I got to know Chase at the Onalaska Y. What a great person….so humble, friendly and genuinely a solid man. I loved to watch him train and guide his son Teague in their power lifting. You could see the pride in his eyes …yet he was so humble…he was a proud Daddy. Chase never said a bad word about anyone and to me…he was like a real live angel living among us. God only takes the best…and Chase was definitely one of them. God bless Chase’s family….you have my deepest sympathies.
