Onalaska- Gina R. Slade, 44, died Monday, May 16, 2011 at Gundersen Lutheran Medical Center in La Crosse.  Gina Rae Matiak was born March 20, 1966.

She is survived by her daughters Cora and Morgan Slade; father Joseph Matiak; sisters, Kristy Engstler (Greg), Julie Strzyzewski (Scott); brothers, Eric Matiak and James Matiak (Paula); and maternal grandmother, Carol Everson.  Gina married Jeff Slade and later divorced.

She is preceded in death by her mother, Geraldine Matiak (May 19, 2011), paternal grandparents, William and Esther Matiak and maternal grandfather, Walter Everson.

Gina was a 1985 graduate of Onalaska High School.  Gina enjoyed swimming, music and sewing.  She lived and worked in Chicago, Orlando, Nashville, and Hilo, Hawaii in the hospitality industry before returning to Onalaska to marry and raise her family.

Thank you to the Gundersen Lutheran ICU Medical Staff and Organ Transplant Procurement Organization.  Gina was able to donate several organs.

A Celebration of Life will be held at the family Boathouse, West Side Boat Club #30 on June 11 from 2:00-5:00 PM.  Coulee Region Cremation Group is assisting the family.
